On Feb. 26, 2019, located at a commercial structure, an employee was working from and operating an elevated SkyJack SJlll-3219 scissor lift without the entry chain (midrail) in place at fall heights greater than 23 feet above the concrete surface below. The employer did not instruct employees in the proper use of a scissor lift (scaffold) and in the recognition and avoidance of those hazards of which a reasonably prudent employer would have been aware of while performing this work.
