29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems: At this worksite, employees engaged in rolling on wet patch leak repair to a commercial roof, were exposed to fall hazards greater then 6 (six) feet to the lower level.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder: At this worksite, employees engaged in roofing activities used a Werner extension ladder to access/egress the roof. The ladder did not extend at least three feet above the roof landing.
