Ave. Commerce City, CO 80022: On or before 2/28/19 the employer did not protect employees from injury in that the Omax Maxiem 1515 (sn#M15155214) and Omax Maxiem 1530 (sn#M15305786) water jets were not guarded or equipped with presence sensing devices. Employees cut composite stock on these water jets on a daily basis. This condition exposed employees to amputation hazards and struck by hazards.
