29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1 for hazards associated with 1) Not instructing employees on the recognition an avoidance of fall hazards and 2) Not ensuring employees used fall protection while working from a ladder jack scaffold.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about February 28, 2019, employees were exposed to a fall of approximately 15 feet while installing siding materials on a residential home located at 22439 Valley Brook, Strongsville, Ohio 44149. The employees were working from a ladder jack scaffold and the employer did not ensure that they were trained on how to recognize and minimize fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 28, 2019, employees were exposed to a fall of approximately 15 feet while installing siding materials on a residential home located at 22439 Valley Brook, Strongsville, Ohio 44149. The employees were working from a ladder jack scaffold and the employer did not ensure that they were using fall protection.
