29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact or from fall or flying objects, or from electrical shock or and burns, were not protected by protective helmets:    (a) Inside UltrePET facility, near maintenance shop and stairs to lab:   On or prior to 2/27/19, employees making repairs to a process sewer line were not wearing protective helmets when working inside excavations, where they were exposed to being struck by falling tools or debris.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a) Inside UltrePET facility, near maintenance shop and stairs to lab:   On or prior to 2/27/19, employees were making repairs to a process water sewer line, working in a trench excavation that was approximately 6' long x 4' wide x 7' deep in type C soil.   No means of egress was located in the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    (a) Inside UltrePET facility, near maintenance shop and stairs to lab:   On or prior to 2/27/19, employees were making repairs to a process water sewer line, working in an excavation that was approximately 6' long x 4' wide x 7' deep in type C soil.   No protective system was in use.
