29 CFR 1926.1412(d)(1): Visual inspections were not performed by a competent person prior to each shift for apparent deficiencies of the equipment that was used:  (a) New residential construction located on Lot #50, Front side driveway: The employer exposed employees to struck-by hazards while installing trusses, in that a competent person did not inspect the crane and related equipment for deficiencies prior to the start of operations on site.

29 CFR 1926.1412(d)(1)(x): The employer failed to inspect ground conditions around the equipment for proper support, including ground settling under and around outriggers/stabilizers and supporting foundations, ground water accumulation, or similar conditions:  (a) New residential construction on Lot #50, Front side driveway: On or about 02/25/2019, and at times prior, the crane operator exposed employees to struck-by hazards while installing trusses, in that the crane was not resting on a firm foundation (blocking, cribbing, and/or mats). Employees were exposed to overhead roof trusses as the crane outrigger pads were not inspected or placed on a firm foundation prior to the start of hoisting operations.
