29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:  a) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that that a personnel platform was designed by a qualified person.  The employer connected a job-made wooden work platform to a Sunstate Equipment rough terrain forklift model 8042 to lift framers to perform exterior work on a three story townhome.  The platform design did not include a complete guardrail system around all sides, a section of the floor was not level and had a 4" x 20" gap, and a toeboard was not installed on the open side of the platform.  The following condition exposed employees to falls from height and struck-by hazards.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.  a) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that that a personnel platform was not moved horizontally while the platform was occupied.  The employer connected a job-made wooden work platform to a Sunstate Equipment rough terrain forklift model 8042 to lift framers to perform exterior work on a three story townhome.  While positioning the platform, the boom was extended horizontally with the worker occupying the platform.  The following condition exposed employees to falls from height.

29 CFR 1926.502(d)(6)(i): Snap hooks were connected directly to webbing, rope or wire rope;  a) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer allowed snap hooks to be directly connected to the rope used for anchorage of a rope grab fall protection system.  Framers were engaged in residential framing activities up to 25 feet and the personal fall arrest system snap hook was looped around a structural beam and attached directly to the rope.  One framing employee was installing trusses and sheathing on a three story residential building while using the rope grab fall protection system.  This system for anchorage had been used and available for all other framers working at the job site. The manufacturer's manual for this fall protection system indicated that this was an inappropriate connection for anchorage of the vertical lifeline. This exposed employees to falls from height.

29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:  a)  Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that that the vertical lifeline used on the Sunstate Equipment rough terrain forklift platform model 8042 was protected against being cut and abraded.  A job made work platform was being used to lift framers to perform exterior work on a three story townhome.  The vertical lifeline rope used for fall protection on the platform was looped around the fork attachment and connected to itself.  The fork attachment that the rope was attached to had an edge which could cause the rope to be cut or damaged during normal use and in the event of a fall.  This condition exposed employees to fall hazards from height.  b)  Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that lifelines were protected against being cut or abraded.  Framers were engaged in residential framing activities up to 25 feet and the personal fall arrest system snap hook was looped around a structural beam and attached directly to the rope.  The rope was fed up to the roof and bent at a 90 degree angle across the edge of a wood beam to the area where the framer had been working.  One framing employee was installing trusses and sheathing on a three story residential building while using the vertical lifeline system.  This system for anchorage had been used and available for all other framers working at the job site.  This exposed employees to falls from height.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet nor contact any lower level:  a) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that personal fall arrest systems, when stopping a fall, were rigged such that an employee could not free fall more than 6 feet nor contact any lower level.  Employees used a Sunstate Equipment rough terrain forklift model 8042 to hoist a wooden job-made personnel platform, and the employees used a shock-absorbing lanyard to connect to the mast.  The employer did not ensure that the personal fall arrest system was rigged in a manner such that the employee could not free fall more than 6 feet not contact a lower level.  This condition exposed the employee to fall hazards.      b) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that personal fall arrest systems, when stopping a fall, were rigged such that an employee could not free fall more than 6 feet nor contact any lower level.  Employees framing the second story and sheathing the roof used a shock absorbing personal fall arrest system anchored to a structural top plate.  This shock absorbing lanyard and anchorage point did not stop the employee from falling more than 6 feet or from coming into contact with the lower level.  This condition exposed the employee to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that training was provided for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Employees were working from a job-made platform connected to a Sunstate Equipment rough terrain forklift model 8042.  The employees were using a shock absorbing lanyard that was looped around a sharp edge for fall protection of the forklift mast.  The employer did not ensure that employees were trained to use the proper fall protection equipment, such as retractable lanyards, to prevent employees from coming into contact with the ground, and to use proper anchorage points to prevent damage to the fall protection equipment.  This condition exposed employees to fall hazards from height.    b) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that training was provided for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. 29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  a)  Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that ladders used for access to an upper landing surface extended at least 3 feet above the upper landing surface to which the ladder was used to gain access.  The ladder used for access extended approximately 15 inches beyond the upper landing and was next to the edge which dropped off approximately 25 feet to the ground. The ladder was not secured and had approximately 6 feet of overlap which could have been used for additional extension beyond the upper landing surface.  One framing employee was installing trusses and sheathing on a three story residential building while using the ladder for access.  This system for roof access had been used and was available for all other framers working at the job site.  This condition exposed employees to fall hazards from height.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  a) Felix Framing located at 208 Galena Street Frisco, CO 80443:  On and preceding 27 February, 2019, the employer did not ensure that each employee on a walking/working surface was protected from tripping in or stepping into or through holes on the third floor of the townhome under construction.  A metal structural beam going from the floor of the third level to the ceiling had several floor holes around the beam which measured approximately 17 inches by 3-6 inches wide.  The framers were working in the immediate area of the floor holes carrying tools and materials.  The following condition exposed employees to tripping and mechanical injuries.
