29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person:     On or about February 25, 2019, in the office of an establishment located at 315 W Ocean Boulevard in Stuart, Florida, an employee was exposed to a fall hazard of approximately 5 feet - 10 inches when performing paintwork from a mobile scaffold that was not erected under the supervision and direction of a competent person in scaffold erection.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:   On or about February 25, 2019, on site of an establishment located at 315 W Ocean Boulevard in Stuart, Florida, an employee was exposed to a fall hazard when performing paintwork from a mobile scaffold prior to being provided with a training program that would enable him to recognize the hazards associated with the scaffold.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:     On or about February 25, 2019, in the office of an establishment located at 315 W Ocean Boulevard in Stuart, Florida, an employee was exposed to a fall hazard of approximately 5 feet - 10 inches when performing paintwork from a mobile scaffold while the casters and wheels of the scaffold were not locked to prevent movement of the scaffold while being occupied.
