29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 12:00 PM on January 30th 2019 at 1249 Drew St, Lakeland, FL 33810, an employee suffered a work related amputation on his left hand middle finger and his left hand ring finger. The employer was aware of the amputations on January 30th 2019. The employer notified OSHA on February 6th at 12:11 PM of the work related amputations.
