29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   On or about February 28, 2019 the employer failed to ensure that an annual audit of the energy control procedures for equipment serviced and maintained in the facility was performed. The employer had an established program including written LO/TO procedures however failed to audit this program annually.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 28, 2019 the employer failed to ensure that the hydraulic tube/ pipe benders were effectively guarded to prevent employees from the hazardous areas of the equipment. Hazardous areas on the equipment include but are not limited to the clamp mechanisms, benders, and other moving equipment. The equipment was operated and bending cycles started by foot pedal. Employees are able to reach into the hazardous areas at any time during operation. The YLM tube bender contained a pressure sensitive floor mat, however this was ineffective in that employees could still reach the hazards.     b) On or about February 28, 2019 the employer failed to ensure that effective guarding was in place on the 120 welding robot cell. During welding operations employees are able to reach the robot and welding operation due to a lack of barrier guarding on the sides of the cells. Employees are protected from the rotation of the production table during movement by the use of light curtains.

29 CFR 1910.217(b)(7)(iii): A means of selecting Off, "Inch," Single Stroke, and Continuous (when the continuous function is furnished) was not supplied with the clutch/brake control to select type of operation of the press. Fixing of selection was not by means capable of supervision by the employer.  On or about February 28, 2019 the employer failed to ensure that the keys for the selection between "Inch," Single Stroke, and Continuous were kept under supervisory control. The keys for this selection were left in the control panel while an employee operated the #1 Rouselle mechanical power press.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):   a) On or about February 28, 2019 the employer failed to ensure that effective point of operation guarding was in place on the backside of the #1 Rouselle mechanical power press.   b) On or about February 28, 2019 the employer failed to ensure that effective point of operation guarding was in place on the backside of the #2 Heim mechanical power press. Simple removable chain link and signage was used on the backside.  c) On or about February 28, 2019 the employer failed to ensure that effective point of operation guarding was in place on the backside of the Komatsu 60 mechanical power press.
