29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Feather Room - An employee was impaled by a stainless steel bar that was thrown from the rotating drum of the extractor.  The employer did not ensure that a guard was installed on the extractor to prevent employees from accessing the drum while in operation, on or before February 26, 2019.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.32(b)(3): A company executive did not examined the OSHA 300 Log to ensure that the annual summary was correct and complete prior to certification.    (a) 2018 OSHA 300A Summary of Work Related Injuries and Illnesses - The 2018 OSHA 300 Log indicated there were zero (0) cases with days away in the total for Column H,  however twelve (12) was entered into the total number of cases with days away from work on the OSHA 300A form, on or about February 26, 2019.       (b) 2018 OSHA 300A Summary of Work Related Injuries and Illnesses - The 2018 OSHA 300 Log indicated there were zero (0) cases of other recordable cases in the total for Column J,  however twelve (12) was entered into the total number of other recordable cases on the OSHA 300A form, on or about February 26, 2019.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
