29 CFR 1926.452(n)(3): Ladders used to support step, platform, and trestle ladder scaffolds shall be placed, fastened, were not equipped with devices to prevent slipping: a) At 970 Lantana Ave., Clearwater, FL, an employee was installing insulation on the second floor of a residence, while working from a plank extension supported by a 6 foot and a 12 foot ladder that were not equipped with devices to prevent slipping. The employee fell approximately 14 feet when the plank slipped from the ladders, on or about 2/12/2019.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards: a) At the renovation project located at 970 Lantana Ave., Clearwater FL, an employee working 14 feet high from a plank extension installing insulation was not trained to recognize the associated hazards by a qualified person, including the fall hazard, on or about 2/12/2019.
