29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person: a) Anchor Roofing, LLC, 3400 Baseline Road, Boulder, CO 80303: On and before February 22, 2019, the employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person. The employer allowed employees to work on a 6/12 pitched roof approximately 80 feet high even though the roof was icy. An employee fell approximately 25 feet when he slipped on the ice. This condition exposed employees to a fall hazard of approximately 25 feet.
