29 CFR 1910.28(b)(11)(ii): Stairways. The employer did not ensure each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails per Table D-2; stair width less than 44 inches (1.1 m), and one open sides required to have one stair rail system on each open side:    a) On or about March 1, 2019 in the blending area, a stairway used by employees to access the blending platform did not have a midrail, exposing employees to a fall hazard.    b) On or about March 1, 2019 in the back dock area, a stairway used by employees did not have a stair rail system exposing employees to a fall hazard.
