29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)   This violation occurred on or about March 1, 2019 at a job site located at 821 Knoxbridge Road, Forney, TX; where employees were working and walking around on the ground and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a)   This violation occurred on or about March 1, 2019 at a job site located at 821 Knoxbridge Road, Forney; where employees were using nail guns to install roofing shingles were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  This violation occurred on or about March 1, 2019 at a job site located at 821 Knoxbridge Road, Forney, TX; where employees were using a 14-feet extension ladder that was broken to make two parts did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard.  (a)   Ladder located on the south side of the house.  (b)  Ladder located on the north side of the house

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  (a)     This violation occurred on or about March 1, 2019 at a job site located at 821 Knoxbridge Road, Forney, TX; where employees doing roofing work at a new residential single family house were exposed fall hazards to the lower level.  CHRISTIAN HECTOR LERA  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUMPATIONAL SAFETY AND HEALTH STANDARD 1926.501(B)(13) OR ITS EQUIVALENT WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 136646 CITATION NUMBER 1, ITEM NUMBER 1A THAT WAS ISSUED ON JANUARY 9, 2019 WITH RESPECT TO A WORKPLACE LOCATED AT 964 KNOXBRIDGE ROAD, FORNEY, TX 75126.  THE FINAL ORDER DATE WAS FEBRUARY 8, 2019.
