29 CFR 1910.146(l)(1): The employer failed to consult with affected employees on the development and implementation of all aspects of the permit space program required by paragraph ( c ) of this section:  (a)    On or about February, 26 2019, and at times prior, for the employees exposed to potential confined space hazards where the employer did not effectively communicate permit and non-permit entry requirements to employees performing clean up on sump 2 at or near 3630 Gateway Drive in Grand Forks, North Dakota.
