29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about February 26, 2019, at the Ensign Rig #121: The sewer pumps for the change house there were connected to the electrical outlet with (3) extension cords which were connected in series (daisy chained) to one another. Employees were exposed to electrical hazards.
