29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) AFAM Concept, Inc., Production Department - Chicago, IL - On or about February 27, 2019, the employer failed to conduct an adequate personal protective equipment (PPE) assessment for employees operating a Double Pouch Machine (Machine #1). Employees were required to clean the blade on the machine and were not provided appropriate PPE, such as cut-resistant gloves, thereby exposing the employees to cut, laceration and amputation hazards.
