29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 1, 2019, on the Production Floor, the employer failed to provide an effective method of machine guarding for employees operating an Orin Pallet Wrapper, Serial Number V2852, to protect employees who operate this machine from ingoing nip points created by the rotating platform. Employees were thereby exposed to amputation hazards.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: Dhaliwal Labs North, LLC - Bedford Park, IL - On or about March 1, 2019, the employer failed to record the following workplace injuries in the detail required by the regulation on the OSHA 300 Log for calendar year 2019. a. On or about February 14, 2019, a recordable injury occurred when a mechanic sustained a burn of the cornea and conjunctival sac of left eye after liquid soap splashed into his eyes while changing the product supply pump, resulting in 3 days of lost time. The information in Column F was not recorded in the detail required, as it did not described the object that directly injured the employee. b. On or about February 20, 2019, a recordable injury occurred when a material handler sustained an amputation of the tip of the right hand thumb when the thumb was caught in a nip point on the Orin Pallet Wrapper, resulting in over 30 days of lost time. The information in Column F was not recorded in the detail required, as it did not described the object that directly injured the employee.
