29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. a. In areas such as but not limited to, the raw materials warehouse and finished good warehouse, permanent aisles and passageways were not marked where there was pedestrian and powered industrial vehicle traffic in the same areas.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. When throwing quilts on the line one and line two Merrello machines, guarding was not in place to prevent employees from entering the danger zone of the moving clamp arms returning to their start position. On or about February 27, 2019, an employee was struck in the face by the returning clamp arms of the Merrello machine on line one. b. When throwing quilts on the line one and line two Merrello machines, there was no guards in place to protect employees from the ingoing nip point of the gears and belts that controlled the moving bridge for the clamp arms located below the shelf where the operated stood.
