Employees handle materials in the yard and warehouse as well as deliver building materials to customers. Employees also mix paint, clean areas of the store and handle returned items. This condition exposed employees to hazards associated with performing job tasks without the required personal protective equipment.

Flexible extension cords were strung throughout the facility and affixed in a permanent manner. Paragraph A prohibits the use of extension cords in lieu of permanent wiring. This condition exposed employees to electrical shock and fire hazards.
