29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a) Production Department - On or about 3/4/2019 and at times prior thereto, the employer exposed employees to amputation and electrical shock hazards in that the employer failed to develop and maintain machine specific lockout procedures for employees repairing machines such as, but not limited to, Hanwha Corporation HCM-3100- E1-#6 and Hanwha Corporation Box Packing machine that had electrical (220-volt), mechanical and gravitational energies.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources.  a) Production Department - On or about 3/4/2019 and at times prior thereto, the employer exposed employees to amputation, caught-in and electrical shock hazards in that the employer failed to provide lockout and tagout devices to authorized employees while employees were required to repair machines such as, but not limited to, the Hanwha Corporation HCM-3100- E1-#6 and Hanwha Corporation Box Packing machines that contained electrical (220-volt), mechanical and gravitational energies.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a) Production Department - On or about 3/4/2019 and at times prior thereto, the employer exposed employees to potential amputation, caught-in and electrical shock hazards in that the employer failed to ensure each authorized employee received lockout training when they were required to repair machines such as, but not limited to, Hanwha Corporation HCM-3100- E1-#6 and Hanwha Corporation Box Packing machine that contained electrical (220-volt), mechanical and gravitational energies.
