29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) National Guard Armory 1337 Highway 98 West, Columbia, MS 39429 - On or about February 27, 2019 the employer did not ensure, by observation or instrumentation, that the Life Safety circuit was de-energized and grounded prior to permitting employee(s) to install light fixtures, exposing the employees to an electrical shock hazard while stripping insulation from wires and connecting circuits to fixture wires.

29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative, and did not have tags attached at all points where such equipment or circuits could be energized: (a) National Guard Armory 1337 Highway 98 West, Columbia, MS 39429 - On or about February 27, 2019 the employer did not ensure circuit #28 was rendered inoperative and had a warning tags in places where the circuits could be energized, while an employee stripped circuit wires and connected light fixtures to the circuit, exposing employee to an electrical shock hazard.
