29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a) Trench located on rear side of 603 Atlanta Street, Roswell, GA: On or about 03/05/2019, and at times prior, the employer exposed employees to cave-in hazards while working in an excavation at depths of 6-feet, 10-inches deep and greater, in that a safe means of egress (i.e. ladder and/or ramp) was not provided while engaged in plumbing activities.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:     (a) Trench located on rear side of 603 Atlanta Street, Roswell, GA: On or about 03/05/2019, and at times prior, the excavation was not inspected by a competent person prior to the start of work, in that the employer did not provide a cave-in protection system, a safe means of egress, and the excavation edge was not free of building materials and spoil piles within 2-feet of the edge of the trench.

(a) Trench located on rear side of 603 Atlanta Street, Roswell, GA: On or about 03/05/2019, and at times prior, the employer exposed employees to cave-in hazards while working in a trench that measured approximately 4-feet wide by 6-feet, 10-inches deep by 17-feet long, in that a cave-in protection system was not provided to an employee engaged in plumbing activities inside the excavation.
