29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Grant Contracting LLC: On or about March 6 2019, the employer did not ensure the side rails of the extension ladder used on the side of the commercial structure was extended at least three feet above the landing surface. Four employee were exposed to falls while using the ladder to access/egress the roof.
