29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about March 6, 2019 at a worksite located 1304 Cornell, Marion, Il. 62959,  the employer did not ensure fall protection was implemented during commercial construction operations when workers were exposed to the hazard of falling approximately 11 feet while installing 50 foot corrugated metal roofing.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Grant Contracting LLC: On or about March 6 2019, the employer did not ensure the side rails of the extension ladder used on the side of the commercial structure was extended at least three feet above the landing surface. Four employee were exposed to falls while using the ladder to access/egress the roof.
