29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6.      At the facility, compress gas and oxygen cylinders not secured from falling exposing employees to struck-by hazards:    1.  Outside the production building on the east side, gas and oxygen cylinders were not secured from falling,    2.  Inside the production area gas and oxygen cylinders not secured from falling.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.      In the production area near the paint booth, the employer did not provide an eyewash/shower station where employees were expose to eye hazards while spraying paint containing category 1 corrosive materials.

29 CFR 1910.305(b)(3)(ii): 29 CFR 1910.305(b)(3)(ii): Boxes shall be closed by suitable covers securely fastened in place, boxes were not closed as required.      In the grinding area, the employer did not ensure a cover plate was place on the square junction box with live wires that energize machinery were effectively closed during operations exposing employees to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).      In the production area, the employees used an extension cord with the outer sheathing split exposing internal wiring exposing them to electrical shock hazards.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use.       On the east side of the production building, an unused oxygen cylinder was stored without the protective cap exposing employees to struck-by hazards.
