The same energy control checklist was being used for all the machines and equipment; exposing employees to ingoing nip-points, electrical shock, and caught-in hazards in the event of un-expected energization while they are performing maintenance such as replacing worn bearings or broken parts.    (c) On or about March 6, 2019, the energy control procedures for a Newfoil Machines Hot Stamping 4500 printing press were not developed, documented and utilized for the control of potentially hazardous energy. The same energy control checklist was being used for all the machines and equipment; exposing employees to burn, ingoing nip-points, electrical shock, and caught-in hazards in the event of un-expected energization while they are performing maintenance such as replacing worn bearings or broken parts.    (d) On or about March 6, 2019, the energy control procedures for a Mark Andy model 2200, 7-inch capacity, printing press (marked press 1) were not developed, documented and utilized for the control of potentially hazardous energy. The same energy control checklist was being used for all the machines and equipment; exposing employees to ingoing nip-points, electrical shock, and caught-in hazards in the event of un-expected energization while they are performing maintenance such as replacing worn bearings or broken parts. 29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) On or about March 6, 2019 it was found that pneumatic cylinder repair or replacement was being done on Mark Andy presses as needed. Prior to doing the work, the electrical power is shut off and a lock and tag is affixed to the electrical disconnect switch. Air pressure is shut off at a ball valve or pressure regulator where supplied air enters the press. An authorized employee affixes no lockout or tagout devices to the ball valves or pressure regulators; prior to the work being done, exposing employees to a pinch point hazard.
