29 CFR 1910.106(d)(2)(i): Approved containers and portable tanks were not used to store flammable materials.    a) On or about 04/11/19 outside the Clean room; employees were exposed to burns when the container used to hold flammable materials ( isopropyl alcohol ) for parts and tools cleaning was missing the support for the fuseable link fire safety feature on the inside of the cover. 29 CFR 1910.147(c)(4)(i): Procedures were not documented or utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 03/04/19 in the Clean Room; employees were exposed to caught between hazards of the upper and lower mold during mold installation on press #31, when the operator's finger was pinched between the upper mold and the lower mold.  Power to the press was "On" with the controls in Auto mode when a misaligned sensor allowed the mold to close pinching the employee's finger between the molds resulting in a lost time incident.     ABATEMENT CERTIFICATION REQUIRED
