29 CFR 1910.178(l)(2)(i)(A): The employer did not ensure trainees operated a powered industrial truck only under the direct supervision of persons who have the knowledge, training, and experience to train operators and evaluate their competence:    On or about 3/1/2019, at the warehouse area of General Hotel & Restaurant Supply Corp, 13900 NW 82nd Avenue, Miami, FL 33116, a trainee was allowed to drive a stand up reach truck (i.e. Crown, Serial #1A196318) without being supervised, which resulted in the employee to struck a column while heading to pickup empty pallets.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:    On or about 3/6/2019, at the warehouse area of General Hotel & Restaurant Supply Corp, 13900 NW 82nd Avenue, Miami, FL 33116, an employee was exposed to a hazardous chemicals including but not limited to, Kitchen Mate oven cleaner ( a corrosive, with a pH of 13.0), ShaZyme DRC, Wrangler Disinfectant Toilet Bowl Cleaner (corrosive, with a pH of less than 2), 20X Glass and Window Cleaner Concentrate (corrosive, with a pH of 11.5), RUFF N Ready Degreaser (corrosive, with a pH of 11), Bullseye Degreaser (corrosive, with a pH of 13), while handling hazardous materials, as he was not knowledgeable of the location and availability of the company's hazard communication program and/or safety data sheets.
