29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about March 13, 2019, the employer did not ensure that employees were protected from fall hazards while walking on the press molds located in press #1. Employees conducted activities such as unhooking the crane hook after the molds are installed. Employees were not protected by methods such as, but not limited to, guardrails or safety net systems or by personal fall arrest systems. Employees were exposed to fall hazards of approximately 11feet to the floor below.
