29 CFR 1910.1200(e)(1):  The employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   (a) Avalon Acoustics at 2800 Wilderness Place, Suite A, in Boulder, CO: On and preceding March 7, 2019, the employer did not develop and implement a written hazard communication program that pertains to employee use of chemicals such as, but not limited to, acetone, isopropyl alcohol, ranger board (formaldehyde), thermo-lite board (fiberglass), gorilla glue (isocyanates), fillers, mineral spirits,  adhesives, solders, glues, veneer softeners and waxes.  This condition potentially exposes employees to chemical irritations to eyes, respiratory, and skin and inhalation hazards.

29 CFR 1910.1200(f)(6):  The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either: (i) The information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers; or, (ii) Product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) Avalon Acoustics at 2800 Wilderness Place, Suite A, in Boulder, CO: On and preceding March 7, 2019, the employer did not ensure that each container of hazardous chemicals win the workplace was labeled, tagged or marked to covey the hazards represented by the employee use of chemicals such as, but not limited to, acetone, isopropyl alcohol, ranger board (formaldehyde), thermo-lite board (fiberglass), gorilla glue (isocyanates), fillers, mineral spirits,  adhesives, solders, glues, veneer softeners and waxes. 29 CFR 1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they are readily accessible during each work shift to employees when they are in their work area(s):    (a) Avalon Acoustics at 2800 Wilderness Place, Suite A, in Boulder, CO: On and preceding March 7, 2019, the employer did not maintain safety data sheets in the workplace.  This condition potentially exposed employees to hazardous chemicals, including but, not limited to, acetone, isopropyl alcohol, ranger board (formaldehyde), thermo-lite board (fiberglass), gorilla glue (isocyanates), fillers, mineral spirits,  adhesives, solders, glues, veneer softeners and waxes.  This condition potentially exposes employees to chemical irritations to eyes, respiratory, and skin and inhalation hazards.  Abatement Note:  Electronic access and other alternatives to maintaining paper copies of the safety data sheets are permitted as long as no barriers to immediate employee access in each workplace are created by such options.

Employees conducted operations which included exposure to irritating fiberglass particles, hazardous wood dust, glues, fillers, solvents and flying particles.  Hazardous operations included, but are not limited to the following:  1.  Exposure to wood board that contains formaldehyde;  2.  Exposure to flying particles when processing wood or plastic materials; and 3.  Exposure to glues, fillers, and solvents while assembling the finished products.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a) Avalon Acoustics at 2800 Wilderness Place, Suite A, in Boulder, CO: On and preceding March 7, 2019, the employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer. The employer allowed employees to voluntarily wear particulate filtering facepiece respirators (dust masks). Employees wore N95 respirators while cutting Thermo-lite board. This condition potentially exposed employees to a respiratory hazard.
