On or about March 7, 2019, at the above addressed jobsite, an inspection was not conducted by a competent person capable of correcting hazards such as falls, which could result in employee injuries.

On or about March 7, 2018, at the above addressed jobsite, an employee conducting siding installation operations from an aerial lift was not provided training on how to minimize fall protection hazards, thereby exposing the employee to the hazards associated with falls.

On or about March 7, 2019, at the above addressed jobsite, an employee performing siding installation operations from a Marklift aerial lift, Model number CH44C, approximately eight (8) feet above the ground, was not wearing a full body harnesses equipped with a lanyard, thereby exposing the employee to falls.
