29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling:    Employees are not protected from fall hazards while working near the automotive service pit in the automotive shop. This was most recently documented on March 6, 2019 at Olson Motor Lines located 21852 R&R Road in Gretna, Nebraska 68028. One employee performs vehicle maintenance at Olson Motor Lines and all other employees have access to the automotive shop. The automotive service pit in the automotive shop is about 39.5" wide and roughly 34' 7.5" long. The service pit itself is approximately 68" deep. There was no form of fall protection system in place around the service pit.

This was most recently documented on March 6, 2019 at Olson Motor Lines located 21852 R&R Road in Gretna, Nebraska 68028. One employee performs vehicle maintenance at Olson Motor Lines and other employees have access to the automotive shop. This was most recently documented on March 6, 2019 at Olson Motor Lines located 21852 R&R Road in Gretna, Nebraska 68028. One employee performs vehicle maintenance at Olson Motor Lines without written specific procedures. There are currently 27 different semi-trucks, and each truck has 3-4 keys.

This was most recently documented on or about March 6, 2019 at Olson Motor Lines located at 21852 R & R Rd., in Gretna, Nebraska. Employees work with and around various chemicals at Olson Motor Lines. Employees work with Diesel Fuel No. 2, which contains naphthalene. Employees involved with vehicle maintenance also work with various automotive chemicals, including Allied Global Heavy Duty ELC 50/50 Extended Life Coolant, Shell Rotella T5 10W-30, Allied 75W-90 Synthetic  Gear Lubricant, Simply Blue+ Diesel Exhaust Fluid, Allied Mega Red LC #2, and Allied Windsheid Washer Fluid, Emgard MTF 7000.
