29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) 342 Shady Point, Alexander City, AL; on or about March 5, 2019 and times prior the employer exposed his employees to fall hazards from a scaffold platform 19.8 feet from the ground level by not providing complete planking of the working surface. The working surface had only three of the five required planks.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling. Note:  See exceptions in paragraphs (b)(3)(i) and (ii) of this section.  a) 342 Shady Point, Alexander City, AL; on or about March 5, 2019 and times prior the employer exposed his employees to fall hazards from a scaffold platform approximately 13.1 feet from the ground level by not providing a working platform less than 14 inches from the face of the work area. The distance from the work face on the second buck was measured at 23 inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a) 342 Shady Point, Alexander City, AL; on or about March 5, 2019 and times prior the employer exposed his employees to fall hazards from a scaffold platform 19.8 feet from the ground level by not providing an access ladder to climb on or off the scaffold.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  a) 342 Shady Point, Alexander City, AL; on or about March 5, 2019 and times prior the employer exposed his employees to struck by hazards by not providing protection for those employees working below the scaffolding from falling brick laying debris to include, but not limited to, bricks, blocks, mortar, and trowels.
