29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about March 7, 2019 and at times prior to that date, for the employees exposed to electrical hazards while preforming work activities where a shop built extension cord was used to energize equipment is the maintenance shop located at building 464, Nellis AFB, Las Vegas, Nevada.     (b) On or about March 7, 2019 and at times prior to that date, for the employees exposed to electrical hazards while preforming work activities where an energize outlet receptacle was installed in a suspended ceiling tile in the maintenance shop located at building 556, Nellis AFB, Las Vegas, Nevada.

29 CFR 1910.305(j)(2)(iv): A receptacle installed in a wet or damp location was not suitable for the location:  (a) On or about March 7, 2019 and at times prior to that date, for the employees exposed to electrical hazards while using equipment connected to a standard electrical receptacle adjacent to a sink and water source and was not ground-fault circuit interrupter (GFCI) protected in the maintenance shop located at building 2907, Nellis AFB, Las Vegas, Nevada.    (b) On or about March 7, 2019 and at times prior to that date, for the employees exposed to electrical hazards while performing work activities on and around an ice machine connected to a standard electrical receptacle adjacent to a drain and water source and was not ground-fault circuit interrupter (GFCI) protected in the first floor vending room located at building 556, Nellis AFB, Las Vegas, Nevada.
