29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Jobsite:  The employees stripping and installing asphalt shingles on the 4 in 12 pitch residential roof were not protected from the hazard of falling approximately 16 feet 8 inches to the ground by the use of guardrails, safety nets or personal fall arrest systems/devices.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that would not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   Jobsite:  The 18-foot aluminum extension ladder used to gain access to the lower roof at a height of approximately 16 feet 8 inches above the ground was not secured at its top and a grasping device was not provided.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.  Jobsite: The employees using the 18-foot aluminum extension ladder to access the roof were not trained by a competent person in the proper procedures to minimize the hazards associated with ladders.
