29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Jobsite:  The employee working from the platform of the pump jack scaffold was not protected from fall hazards by the use of appropriately installed guardrails or personal fall arrest systems.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector was attached;(iv) to a horizontal lifeline; or (v) to any object which was incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  Jobsite: The snap hook on the rope grab of the personal fall arrest system worn by an employee was connected directly to the running line portion of the lifeline, exposing the employee to the hazard of falling due to the cutting of the lifeline.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Jobsite: The personal fall arrest system worn by the employee working from the platform of the pump Jack scaffold was rigged in such a manner that the employee could free fall more than 6 feet and/or hit the ground.
