29 CFR 1910.1000(a)(1): Employee(s) were exposed to an airborne concentration of Carbon Monoxide listed in Table Z-1 in excess of the ceiling concentration of 200 ppm, and time weighted average of 50 ppm.:  a) At the garage #9 of H-46, employee was exposed to 60.7 ppm of carbon monoxide (CO) while pressure washing a stairwell with the Comet ZWD 4040 Power Washer, SN 47261065, on or about March 2, 2019.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about March 2, 2019, an employee suffered a work-related injury and was hospitalized.  The employer was aware of the in-patient hospitalization on March 2, 2019.  The employer notified OSHA on March 5, 2019 at 8:42A.M. MDT, of the in-patient hospitalization.
