29 CFR 1910.106(e)(6)(i):  Adequate precautions shall be taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.  On or about March 4, 2019, at the Pioneer Natural Resources tank battery #7, Sale Ranch 37-17 Stanton, Texas.  Employees were exposed to fire and explosion hazards during work activities at the site.  The employer allowed spark producing equipment to be operated in a hazardous location:  a)  a Skyjack, Model SJ45T, Serial No. 98001300.

"No Smoking" signs shall be conspicuously posted where hazard from flammable liquid vapors is normally present:  On or about March 4, 2019, at the Pioneer Natural Resources tank battery #7, Sale Ranch 37-17 Stanton, Texas.  Employees were exposed to fire and explosion hazards during work activities at the site. 29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  On or about March 4, 2019, at the Sale Ranch 37-17, tank battery train #7.  The employer failed to provide and ensure the use of flame-resistant treated clothing (FRC) by employees, necessary to protect them from burns due to potential flash fires.

Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets:  On or about March 4, 2019, at the tank battery train #7, employees performed lockout and tagout and shut down procedures on the heater treater, oil boot, level controller/float and peanut valve.  The employer did not ensure the employees were provided adequate knowledge and training to recognize the chemical and thermal energy hazards they were exposed too.
