29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment to maintain its mechanical integrity did not follow recognized and generally accepted good engineering practices: a) Zone 1, F.C.C. Unit: On or about March 8, 2019 and at times prior, the employer did not implement interim risk mitigation measures to address deficiencies and potential hazards, including, but not limited to, failure of the E-0310A TO D-0305 3"-HL-03446-N-B2-1 line. The removal of E-310A from service created a deadleg in the process.
