29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about March 7, 2019 the employer failed to ensure that machine specific procedures were utilized on mechanical power press #21 during cleaning and change over operations. Employees were permitted to perform these tasks without the use of LO/TO. Hazardous energies present on the equipment includes but is not limited to gravitational, pneumatic, mechanical, and electrical energy sources.

On or about March 7, 2019 the employer failed to ensure that the keys for the selection of the operation mode of press #75 and press #4 were in the control of or supervised by the employer. 29 CFR 1910.217(c)(1)(i): The employer did not provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. On or about March 7, 2019 the employer failed to ensure that point of operation barrier guarding present on mechanical power press #75 (komatsu 80) was effective in preventing employees from reaching the hazardous area. The distance to the point of operation on the barrier guard was 24 inches and contained an opening of 3 and 15/16 inches.
