29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: Note: Exceptions are provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section:  On or about February 27, 2019, employees were dismantling tubular welded scaffolding wrapped around an elevator shaft of the apartment complex parking garage by stacking the scaffold planks all on one frame on the 6th tier, exposing employees to the hazard of being struck by the scaffold materials and/or falling from the scaffold.

29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  On or about February 27, 2019, employees were dismantling tubular welded scaffolding wrapped around an elevator shaft of the apartment complex parking garage without using employees trained in erecting and disassembling scaffolding, exposing employees to the hazard of being struck by the scaffold materials and/or falling from the scaffold.
