29 CFR 1926.1153(g)(1): The employer did not establish and implement a written exposure control plan:    a) Suburban Terrazzo Tile, Incorporated, Alsip, IL.- On March 8, 2019, at Republic Bank, Lincolnwood jobsite, the employer did not establish and implement a written exposure control plan when grinding and polishing operations were performed on terrazzo flooring containing 10% crystalline silica, and  when working with Lehigh White Portland Cement containing up to 5% crystalline silica.    In accordance with 29 CFR 1903.19(c), abatement  certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met, and which address the requirements outlined in paragraphs (e)(1)(i)-(e)(1)(ii) (Construction Reference: 1926.59):    (a) Suburban Terrazzo Incorporated, Alsip, IL-On March 8, 2019, at the Republic Bank, Lincolnwood, IL jobsite, the employer did not develop, implement and/or maintain at the workplace a complete and effective written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would at least describe how the program elements such as labeling requirements, maintenance and availability of Safety Data Sheets (SDSs), employee training on the hazardous chemicals in their workplace, and maintaining a chemical list would be met.    Employees were exposed to hazardous chemicals, including, but not limited to 2-butoxy ethanol, n-methyl pyrolidone and trimethylamine in the Terroxy Resin Systems Urethane Sealer, 70% isopropyl alcohol, 5.25-8.25% sodium hypochlorite in Clorox bleach, and White Portland Cement which contains up to 5% crystalline silica, and trace amounts of sodium and potassium sulfates and hexavalent chromium.      In accordance with 29 CFR 1903.19(c), abatement  certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
