29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On March 11, 2019, there were no frequent and regular inspections being performed by a competent person in that:    (a) a personal fall arrest system was being used by an employee 21 feet away from its anchorage at a fall height less than 13 feet from the roof edge, allowing the employee to swing fall into the ground below      (b) one in-use self-retracting lifeline being used as a fixed foot-level anchorage was observed lying on its side on the roof structure, adversely affecting total fall clearance    (c) one employee was performing roofing work at the unprotected edge while wearing a harness that did not have the chest straps attached and was not tied off     (d) one employee was observed walking the length of the roof structure with unprotected sides or edges, which was estimated at 75 feet long, while wearing a harness and not tied off     (e) one reusable roof anchor installed on the south end of the roof structure was observed with only one screw attached    (f) two synthetic web sling anchorages on the north end of the upper roof structure were observed wrapped around the sharp edges of a concrete roof structure without sling protection (to prevent scuffing or abrasion due to friction)

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On Mar. 11, 2019, located on the southeast side of a commercial structure, employees were working from a low-slope roof with unprotected sides and edges while not utilizing a fall protection system. Potential fall heights were measured to be greater than 12 feet above the ground below.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall:  be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level;    On Mar. 11, 2019, located on the southeast side of a commercial low-slope roof structure, an employee using a shock-absorbing personal fall arrest system with self-retracting lifeline was performing work while positioned 21 feet away from the fixed foot-level anchorage. The employee was exposed to a fall of approximately 13 feet and the hazard of contact with the ground below.
