29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Job site, 339 Mansfield Avenue, Pittsburgh, PA: On or about March 12, 2019, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were in use on a steep pitched roof section of the building where fall hazards of up to approximately 13 feet existed.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid pport that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 339 Mansfield Avenue, Pittsburgh, PA: On or about March 12, 2019, during roofing activities, the employer did not ensure that a ladder used for access to/egress from the roof was properly extended above the roof line in that it was extended approximately 2 feet above the roof line, exposing employees to fall hazards of up to approximately 13 feet.
