29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       a) East panel of garage roof, on or about 3-12-19:  Two employees were working on a sloped roof (8:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 11 feet 3 inches.       b) South panel of main roof, on or about 3-12-19:  Two employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 11 feet 6 inches.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:       a) Entire roof, on or about 3-12-19:  Three employees were performing roofing on a steep roof, without the training necessary for the employees to recognize the hazards associated with falls and their work, exposing the employees to a fall hazard of 11 feet 3 inches and 11 feet 6 inches.       ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       a) East side of building, on or about 3-12-19:  Two employees were using a portable aluminum multi-function ladder for roof access/egress, with side rails that were 18 inches above the landing surface, exposing the employees to a fall hazard 12 feet 5 inches.
