The foreman and two employees were not wearing eye protection while using pneumatic nail guns.

The employees were observed on an 8/12 sloped roof approximately eight feet above the ground.

The foreman and three employees were using a ladder to access the 8/12 sloped roof. The ladder side rails were not extended three feet beyond the landing or edge of the roof.
