On or about March 12, 2019, during the installation of roofing material at a job site located at 27555 Center Ridge Rd, West Lake, Ohio 44145; the employer did not ensure employees working on the roof of a new school were protected from a fall of approximately 14.8 feet by utilizing a fall protection system.
