29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      On or about March 12, 2019, during the installation of roofing material at a job site located at 27555 Center Ridge Rd, West Lake, Ohio 44145; the employer did not ensure employees working on the roof of a new school were protected from a fall of approximately 14.8 feet by utilizing a fall protection system.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:     On or about March 12, 2019, during the installation of roofing material at a job site located at 27555 Center Ridge Rd, West Lake, Ohio 44145; the employer did not ensure employees working on a low slope roof were protected from a fall of approximately 14.8 feet by utilizing a fall protection system by providing fall protection training or fall hazard recognition.
