29 CFR 1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  For Citation 1, Item 2, issued on 3/26/2019 with an abatement date of 4/30/2019, the employer did not certify the complete abatement of the cited violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about March 12, 2019, at the jobsite above, the employee of Samuel Serrano was exposed to serious fall hazards while installing a roofing system on a residential property.  The employee was exposed to falling up to 18.4ft to the concrete grade without the usage of proper fall protection devices.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On or about March 12, 2019, Samuel Serrano did not ensure that his employee were trained to recognize and minimize the exposure to fall hazards.  The Employee was exposed to falling up to 18.4ft to the concrete grade without the usage of fall protection devices.
