Leech Industries, Inc., Building 3 Die cast area, on or about March 12, 2019, and times prior thereto: Employees did not utilize a face shield and/or similar face protection during work tasks including, but not limited to adding zinc ingot bars into the molten zinc pots of the die cast machines.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a.) Leech Industries, Inc., Building 3 Die cast area, on or about March 12, 2019, and times prior thereto: Employees worked with and around corrosive chemicals including, but not limited to, muriatic acid. Employees did not have access within the immediate work area to an eyewash/shower station.
